Debriefing and Informed Consent
Thank you for participation in the Competitive Streak Consulting (CSC) research investigation that hopes to illuminate the extent to which substance and alcohol abuse has impacted the lives of those working within the financial services industry on a nationwide level. The criteria for participation in this investigation are to work within the financial services industry in any capacity. We urge you to respond to the items in an honest manner, so that our findings will be helpful and informative for society. The risks inherent in participating in an internet investigation are minimal, as we collect only general demographic information and keep no identifying information whatsoever. Your name will not need to be supplied for the purposes of this investigation and the only information collected are the questions asked and the date that the test was taken. Your responses will be compared against employees on a nationwide scale, and many of our findings will reflect an average of substance and alcohol use within the financial services industry. Thus, individuals will not be singled out. We will be asking you a series of demographic questions to better understand you as an individual. Upon completion of these questionnaires, your scores will be computed, and you will be given feedback on whether or not your substance/alcohol use is problematic, and where to turn if you feel that you need help.
I have read and understand the above debriefing regarding the risks inherent in my participation. I am aware that if I find my feedback to be troubling, that there will be referrals for me to contact for the purpose of receiving treatment or counseling.
